Transplant Readiness and Anticipatory Care (TRAC) Program

The TRAC program at Lucile Packard Children’s Hospital Stanford provides specialized comprehensive care to children with stage 3-5 chronic kidney disease (CKD). The team provides patients and families with the tools to make educated decisions about their own healthcare to empower them to make well informed medical decisions, to facilitate self-care and self-efficacy, and to keep them on track for a successful kidney transplant.

TRAC Program Goals

Transplant readiness

  • Educate and discuss options for living, deceased, and National Kidney Registry exchange kidney donor transplantation
  • Prepare individualized pre-transplant roadmap to guide smooth transition from CKD to transplant

Dialysis readiness

  • Introduction to pediatric dialysis
  • Education on dialysis modality months before dialysis starts
  • Interdisciplinary team approach to providing dialysis modality education
  • Advance dialysis access planning

Innovation & Research

The TRAC team has been a leader in nephrology and kidney transplant research for decades. They are currently involved in the following clinical trials and studies:

  • Clinical Trials in Transplantation in Children (CTOTC): Multi-center study to evaluate immune development and alloimmunity in pediatric kidney transplant recipients
  • Raptor study: Multi-center study to evaluate twice-daily formulation of drug cysteamine in patients with nephropathic cystinosis
  • North American pediatric renal trials and collaborative studies
  • Randomized double blind study evaluating effect of aminophylline on GFR post deceased donor renal transplant
  • Belatacept use in kidney transplant patients
  • Cytokine profiling and viral infection and PTLD
