For Referring Providers

Our genetics care team offers consultations and works closely with patients and families to provide a definitive diagnosis. Our robust team of providers are experts in a variety of areas of clinical genetics and genomics, including rare and undiagnosed diseases, neurodevelopmental conditions, vascular anomalies, cardiovascular disorders, newborn screening, and more.

We provide comprehensive care, including complete diagnostic evaluation, clinical management and genetic counseling by a medical geneticist and a genetic counselor.

General Referral Information

Please complete the Genetics Referral Form and submit to https://mdportal.stanfordchildrens.org or call our Referral Center at (800) 995-5724

Additional Genetics Referral Information

If any prior genetic testing has been completed, a copy of test results must be included with the referral. Patients will not be scheduled without this information. 

If a family member has been seen in our clinic, please note the family member’s name and date of birth on the referral form.

Referral for a current/future pregnancy, reproductive counseling, and/or carrier screening?

Referral for a personal/family history of cancer?

Referral for a personal/family history of cardiomyopathy or arrhythmia?

All other indications will be triaged for scheduling in our clinic as appropriate.
