Our philosophy emphasizes taking the time to assess each child individually, from birth through their teenage years. Our board-certified physicians focus on building long-term relationships with families in a warm and welcoming environment. Also, our centralized location provides access to Stanford Health Care Tri-Valley Urgent Care Center. We welcome new patients and look forward to meeting your family.

CHADIS Login

CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Returning Users: Log in with your username and password.

If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time in order to complete the health questionnaires in our office.
