CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

Returning Users: Log in with your username and password.

If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time in order to complete the health questionnaires in our office.

Take exit 17 for Mowry Avenue toward Central Fremont. Use the left two lanes to turn left onto Mowry Avenue. Make a U-turn at Civic Center Drive. 2299 Mowry Avenue will be on your right.

From I-880 North
Take the Mowry Avenue exit toward Central Fremont. Use the right two lanes to turn right onto Mowry Ave. Make a U-turn at Civic Center Drive. 2299 Mowry Avenue will be on your right.

From I-680 South
Take exit 16 for Mission Boulevard/California 238. Turn right onto Mission Blvd. Use the left two lanes to turn left onto Stevenson Blvd. Turn right onto Civic Center Drive. Turn left onto Mowry Avenue. 2299 Mowry Avenue will be on your right.

From I-680 North
Take exit 15 for Washington Blvd toward Irvington District. Keep right at the fork, follow signs for Irvington Drive and merge onto Washington Blvd. Slight right onto Fremont Blvd. Turn right onto Stevenson Blvd. Use the left two lanes to turn left onto Civic Center Drive. Turn left onto Mowry Avenue. 2299 Mowry Avenue will be on your right.
