CHADIS Login

CHADIS is an online system that delivers developmental and health questionnaires directly to parents and families before their visit at our office. After you complete the questionnaire, CHADIS results come directly to your doctor. The results of the CHADIS questionnaires will help us address any concerns about your child’s health and development.

How to Login

New Users: Enter our office phone number as the invitation code 9257558500
Returning Users: Log in with your username and password.
Log in to CHADIS: https://www.chadis.com/chadis/respondent/welcome.do

If you are unable to complete CHADIS before your visit, please arrive 15 minutes before your appointment time in order to complete the health questionnaires in our office.

Follow I-580 E to W Grant Line Rd. Take the Grant Line Rd. exit from I-580 E. Take Mountain House Rd. and Co Hwy J4/Byron Hwy to Vasco Rd. in Contra Costa County. Continue on Vasco Rd. to Brentwood. Continue on Balfour Rd. Drive to Cortona Way. Turn left onto Balfour Rd. Take the first right onto Cortona Way. 100 Cortona Way will be on your right.

From US Highway 580 West
Take the Vasco Rd. N exit. Turn right onto N. Vasco Rd. Continue onto CA-4. Turn left onto Balfour Rd. Take the first right onto Cortona Way. 100 Cortona Way will be on your right.

From CA-4 East
Merge onto CA-4. Turn right onto Balfour Rd. Take the first right onto Cortona Way. 100 Cortona Way will be on your right.

From CA-4 West
Turn left onto CA-4. Turn right to stay on CA-4. Turn left onto Balfour Rd. Take the first right onto Cortona Way. 100 Cortona Way will be on your right.
